The registration deadline is December 12, 2012. In order for your information to appear in the printed directory, your registration form and payment must be received by this date. Registrations received after December 12th, will appear in the Midwinter Addendum.
The Midwinter Directory will reflect the information you provided on your registration form or entered online. This information will include your name, title, company address and phone number as it appears in your confirmation letter. Please take a minute to proof this information carefully for accuracy. If a change is needed, please notify Deborah Stewart by email or fax to 202.220.0830 immediately. The deadline for Directory edits is December 12, 2012.
Please be advised that your Midwinter registration fee is non-refundable after December 12, 2012. Refund of the registration fee, minus a $75 processing fee, will be made for any cancellation if received in writing to firstname.lastname@example.org or fax 202.220.0830 by December 12, 2012. Because of the firm commitments of conference arrangements, FMI can not refund the registration fee for any registrations or cancellations received after December 12, 2012. However, registrations can be transferred ton another company employee.
